Four forms to be signed. This is after our Zoom meeting with both Disclosure Guides & couple. This is to be filled out if you all are ready to fully commit to working with both professionals. This is the paperwork portion of making sure we would be able to communicate about the disclosure case and process. Needed so that we can discuss timelines, the disclosure document and collaborate. 1. For couple: Disclosure/polygraph Agreement
This will be signed by the couple. This is an agreement understanding and agreeing to the pro, cons and possible outcomes of a disclosure.
